NOTE: Before you begin this process, please note that in order to keep our fees as low as they are, we do not accept "health" insurance nor we treat anyone for injuries resulting from accidents that involve workman's compensation, insurane companies, or attorneys.
New Patient Instructions

Download the New Patient Paperwork which includes the Mandatory orientation quiz on page 4 or complete the paperwork online electronically with the link we will send you after you call to schedule your first visit. (Optionally you may watch the second video, download and complete the additional Orientation Quiz to bring with you to the office if you want the lower first visit fee of $99). If you choose only to watch the mandatory 10 minute video, the first visit fee will be $120.

Watch the corresponding orientation video(s), answer the questions on the quiz as you are watching it and bring the quiz(s) with you to the office. Watching the orientation video is Mandatory. If you choose not to watch the 10 minute video, please choose one of the other great chiropractors in your area.
